She can also make sure that your loved one's medication is being served on time, monitoring the condition of your loved one and alerting the medical team to any changes.

When you engage us for hospital bedside nursing, you have the option of extending care from hospital into your loved one's home. This is to ensure that your loved one transitions home safely and minimise the chances of an unplanned rehospitalisation.
Bringing Care Home
When you extend nursing care from hospital to home, our nurse care coordinator will visit your loved one in the hospital to assess and advise on what home care supplies would be needed to prepare for our loved one to return home safely.
We are also able to provide home environment evaluation to provide recommendations on home modifications for your loved one's safety at home.
Our nurse care coordinator will also work with our hospital bedside nurses to draw up a home nursing care plan for your loved one for a smooth transition.
